It sounds like some of her yelling and criticism may be warranted. Your hearing problem could be perceived as ignoring her, your lack of memory causes you to not do all that is required of you,etc I think you need to sit down and discuss your problems with her and ask how you can improve. If you go to the union or to her boss I think you will lose and could be terminated.
